index화면에서 간단한 상담신청에 유용한 소스를 적용해보다 에러가 발생되어 질문 드립니다.

해당스킨을 적용해서 불러오면 작성화면이 뜨는데요. 작성 후 신청을 눌리면 아래와 같은 에러메시지의 화면이 나옵니다.

form_ok.php 파일의 문제인것 같습니다.
